These terms govern your use of the Physio Premium Hub website and services. By making a booking or using this site, you agree to these terms.
1. Bookings
Appointments may be booked online, by phone or by email. A $30 booking deposit is required to secure your appointment and will be deducted from the total consultation cost. Your appointment is confirmed once the deposit has been received.
2. Cancellations & no-shows
We ask for at least 24 hours' notice if you need to cancel or reschedule. Cancellations with less than 24 hours' notice or failure to attend may incur a $50 fee. Cancellation and no-show fees are generally not claimable through private health insurance and remain the patient's responsibility.
3. Fees & payments
Physio Premium Hub is a card-only clinic. Payment is required at the time of your appointment and cash is not accepted. Any outstanding amount, including an applicable cancellation or no-show fee, remains the patient's responsibility.
4. Private health insurance
Physiotherapy may be eligible for a rebate through private health insurance with extras cover. Rebates, annual limits, waiting periods and eligibility vary by policy. Please confirm your expected rebate directly with your insurer before your appointment and bring your health fund details or card. You are responsible for any gap or amount not covered.
5. Appointment timing
Please arrive on time. A late arrival may shorten your appointment to avoid delaying the next patient, while the standard consultation fee will still apply. Approximately five minutes of a scheduled consultation may be reserved for clinical documentation and preparation or delivery of your individual exercise or rehabilitation program.
6. After-hours appointments
Appointments outside our standard business hours incur an additional 20% of the standard consultation fee. The applicable fee will be confirmed when the appointment is booked.
7. Reports and documentation
Physiotherapy, progress, functional and NDIS review reports, support letters and other requested clinical documentation are paid services. Fees start from $94 and vary according to the type, complexity, clinical information and preparation time required. The fee will be discussed and agreed before preparation begins.
